When a Methuselah make a guess to the identity of the betrayer what is
being guessed?
The name of the vampire? ............ex. "Gilbert Duane"
-or-
The uncontrolled vampire card? .......ex. "That one second from the
left."

Name of the vampire, not its position in the uncontrolled region.

I disagree - my group of Methuselahs play that you point to the
uncontrolled vampire that you think would be contested if brought out. I
don't have the card with me right now, but if I remember right the card is
played on your opponent's vampire (so they KNOW what the name is). Thus,
they only have to pick which of your uncontrolled vampires is the same as
that one. This would get easier as the game progresses, unless you use
many Effective Managements, or Kindred Intelligence.
Of course, if I am wrong in my memory, and you play it on your own
uncontrolled vampire, then they would have to give the name of their own
controlled vampire that they think would be in contestion. This is easy
if they have few vampires out.
Ill have to go and check my deck to make sure.

Cards never, ever interact with uncontrolled vampires unless they
specifically say so. Sabine doesn't say she can give blood to an
uncontrolled Tremere, so she can't.
