Does this mean you *have* to prevent one damage if you are able to? Or can
you abstain?

Hmm. The text doesn't use the "may" template, so it must be mandatory.
If you take damage in the first round, you must prevent one point of
it.

But does this mean that you can "save" the prevention for later that round, or
do you have to prevent the first damage that comes your way? If it's the
former, then you have the problem that no additional damage is dealt and you
can't retroactively prevent it so I'm guessing you have to prevent the first
damage you take?

No need to guess there - that's what you quoted - if you take damage, you must
