Baseball Bat (new equipment/weapon) says "Do not replace until the end of this action". Suppose you play the card while no action is pending. Do you immediately replace the card? Or never replace the card?

I understood that. My question, and my examples, assume there is no action currently being resolved. Normally, whenever there is a combat, there is an action being resolved. The example I cited was an exception.

The example I cited was an exception. Brujah Frenzy is not an exception to combat occuring during an action. The action is unsuccessful, and there's a combat in the action.
