The limit is per action, yes.
A bleed of >2 simply causes the target Meth. to burn 2 pool (the limit
under Protected Resources).
The bleed itself may be for any amount, and may be increased
arbitrarily. The resulting pool loss is what is limited by PR.
The bleed is not reduced (by official ruling - not obvious from card
text), but rather just the pool loss. This helps keep card interactions
(Archon Investigation, Telepathic Counter) simple.

No. Just as you are not obliged to use any location (Backways, KRCG,
