So in the new Storyline event, each game starts with a Codex of the
Edenic Grounds Keepers in the middle of the table. That you can take a 0
stealth action to equip with for free. Once someone has it, it can't be
stolen by default or anything, right?
So the first guy to take an action to steal it keeps it unless something
wacky happens (like equipment getting stolen with a Fast Hands or
whatever, or getting torped and diablerized or something), correct?

Codex is not weapon, so Fast Hands does not work in its case.

Correct. The default action to take the Codex can only be taken when
the Codex in uncontrolled in the middle of the table.

What happens to the Codex when the minion controlling it leaves the
game (either by being burned or removed from the game)? Does it also
burn/remove itself from the game, or does it go back to the middle of
the table?

It's removed from the game, and never returns. There is no rule that
state 'put it into play (in the middle of the table) after it's
removed from the game', so we could safely assume it's meant to leave
play.
In situation like remove from the game to return latter (descent into
darkness), we could safely assume it's removed, and when minion with
this equipment decide to came back - it simply came back on this
minion.
It's quite logical, but i might be wrong. LSJ know the answer surely.
